NHS Tameside and Glossop Community Heart Failure Service Launch
Segaris were commissioned to brand, launch and market the new NHS Tameside and Glossop Community Heart Failure Service.
The project was broken down into four areas:
1. A Direct Mail campaign to GP's and Practice Nurses across the locality. Consisting of a Service Branded
Noteblock with contact information, packed in a Branded Box with an Introductory Leaflet and Letter welcoming them to the Service. The campaign was very successful and the patient uptake to the Service increased dramatically.
2. Patient information, consisting of an A5 24pg booklet detailing information about the condition, how the service works and what is offered, self-management skills, useful contacts and much more. Bespoke Illustrations were created for the Service to illustrate the Self-Management skills pages.
3. Information for Clinicians to follow up the DM Campaign. Consisting of an A4 Folder + Business Card. Containing an A4 4pg Leaflet with full Service Information, Letter, Referral Form, Feedback Form and A3 Patient Pathway poster.
4. The Community Heart Failure Website, which was for both general public and Healthcare professionals; and was produced following all the NHS Guidelines for both design and accessibility. The website was content managed and staff were trained to manage and update the site.
